Requirements for Certificate of Insurance

– For general contracted work, the vendor shall obtain and maintain in full force and effect commercial general liability insurance for bodily injury and property damage with a combined single limit of not less than $1,000,000 each occurrence.

– If you cannot provide a combined single limit of $1M for each occurrence for primary coverage, then the Additional Insured Language should state the following: “Umbrella liability coverage is primary and non-contributory to all other insurance afforded the additional insured.”

– When the service includes any transportation services the Vendor shall obtain and maintain in full force and effect Commercial Automobile Liability Insurance for all owned, non-owned and hired autos with a combined single limit of not less than $5,000,000 per occurrence.

– The Vendor shall obtain and maintain in full force and effect Statutory Worker’s Compensation and Employer’s Liability Insurance as required by NY State or any other state to which they are required to maintain worker’s compensation and employer’s liability insurance.

– The Vendor shall name the Customer, as additional insureds to the general liability insurance and automobile liability insurance policies written as follows:
“The Trustees of St. Patrick’s Cathedral in the City of New York, Calvary Cemetery, Gate of Heaven Cemetery, Ascension Cemetery, Resurrection Cemetery, St. Mary’s Cemetery, The Church of St. Ann, St. Patrick’s Cathedral, The Basilica of Old St. Patrick’ Cathedral, Ecclesiastical Properties Corporation, The Parish Cemetery Management Company, Archdiocese of New York and His Eminence Timothy Cardinal Dolan are named additional insured.”

– Each policy must contain endorsement that such policy is primary and noncontributory insurance to any other insurance available to the additional insured with respect to claims arising here under.

– Once again, name as additional insured:
“The Trustees of St. Patrick’s Cathedral in the City of New York, Calvary Cemetery, Gate of Heaven Cemetery, Ascension Cemetery, Resurrection Cemetery, St. Mary’s Cemetery, The Church of St. Ann, St. Patrick’s Cathedral, The Basilica of Old St. Patrick’s Cathedral, Ecclesiastical Properties Corporation, The Parish Cemetery Management Company, Archdiocese of New York and His Eminence Timothy Cardinal Dolan are named additional insured.”

– Each policy must expressly state that it may not be cancelled or non-renewed unless thirty (30) days’ advance notice of cancellation is given in writing to Customer by the insurance company.
